Dear Members, 
We need a survey to know your intent for attending the New Years Eve Party as well as any feedback you would like to share.  If you plan to attend or think you will purchase tickets (no commitment at this time), please simply RSVP with your name and any additional guests that will attend with you.  We would like to get an estimate of how many members and guests plan to attend.  We are not asking you to purchase tickets at this time, rather just getting an idea of how many may attend.  Also if you have any comments, please feel free to add them while making your RSVP.   Your input and feedback is very valuable at this time.  Thanks in advance for taking this survey!
